Arizona Notice Of Filing Guardian And/Or Conservator Report is a document that is filed with the court when a guardian or conservator has been appointed to manage the affairs of an incapacitated person. It provides notice to all parties, including the incapacitated person's family, of the appointment of the guardian or conservator. This report must be filed with the court within 10 days of the appointment and must include the name and contact information of the guardian or conservator, the name and contact information of the incapacitated person, the date of the appointment, and a summary of the guardian or conservator's duties.
