Alaska Offer of Proof — Witnesses, SHC-1081 is a form used by attorneys in Alaska to enter into evidence a witness or witnesses' testimony during a court hearing or trial. The form is used to present evidence to the court in the form of documents, exhibits, or sworn statements from a witness. The form is also known as the Alaska Witness Declaration Form or the Alaska Offer of Proof Form. The form is divided into five sections: (1) Identification of Witness; (2) Offer of Proof; (3) Statement of Evidence; (4) Sworn Statement; and (5) Certificate of Service. The form is available in both English and Spanish. There are two types of Alaska Offer of Proof — Witnesses, SHC-1081 forms: one for civil proceedings and one for criminal proceedings.
