Alaska Paternity Witness Statement is a document that is required by the state of Alaska when a man is attempting to establish paternity of a child. This document allows a man to legally establish himself as the father of a child born out of wedlock. It is important for the man to sign a Paternity Witness Statement in order to establish his rights and responsibilities for the child. There are two different types of Alaska Paternity Witness Statement. The first type is known as an Acknowledgement of Paternity, which is used when the mother and the father agree to the paternity of the child. The second type is known as a Denial of Paternity, which is used when the man denies that he is the father of the child. The Paternity Witness Statement must be signed and witnessed by two witnesses, who must be over 18 years of age. The statement must also be notarized by a notary public. Once the statement is completed, it is filed with the Alaska Office of Vital Records, which will then add the father's name to the child's birth certificate.
