The Alaska Appellant Brief Cover Sheet — for Appeals from Administrative Agencies is a document that is used to prepare and submit an appeal to the Alaska Appellate Courts from a decision made by an administrative agency. The Cover Sheet includes information such as the name of the Appellant, the name of the administrative agency from which an appeal is being made, the cause of action, the date of the decision, the date of the appeal, and the names of the attorneys involved. The Cover Sheet also includes instructions about where to file the appeal and how to serve copies to the other party. There are two types of Alaska Appellant Brief Cover Sheet — for Appeals from Administrative Agencies: an Original Cover Sheet and a Reply Cover Sheet. The Original Cover Sheet is used to initiate an appeal and the Reply Cover Sheet is used to respond to an opposing party’s appeal.
