Certificate of Service

The Louisiana Certificate of Service is a document used to prove that a particular legal document was served to the other party in a legal case. The Certificate of Service is signed by the person who served the document and includes a statement that the document was served to the other party in accordance with Louisiana law. There are two types of Louisiana Certificates of Service: one for service by the sheriff and one for service by a non-sheriff. A Sheriff Certificate of Service is used when the document is served by the Sheriff's Office, and a Non-Sheriff Certificate of Service is used when the document is served by someone other than the Sheriff. Both certificates must be completed accurately and include the date and time of service and the name and address of the person served.
