Review all passport application requirements before booking an appointment. Please arrive 10 minutes early with your ID, payment, and all required documents. A photocopy of the front and back of the Proof of Citizenship document is required. And no passport is complete without a USPS Passport Wallet. Use our Form Filler to fill out your form and print it. Application form DS-11: This form should be filled out but not signed until you submit it at the post office. You need to book at either midnight when appointments are released or 5-6am and you'll get appointments for exactly one month later.
