All passengers are required to fill out an E-Ticket or paper form when entering or exiting the Dominican Republic. If using E-Ticket, a new form is required for each entry and exit and the code generated upon form completion can be presented at the airport on a digital device.

Travelers need a QR Code to enter Dominican Republic, which is generated through the government's e-Ticket platform. This system makes entering the Dominican Republic easier by combining customs, immigration, and health forms into one QR Code. Travelers show this code at the security check-ins when they arrive.
First make sure you have your eTicket filled out prior to arrival (I submitted mine 3 days before my flight left home). Your eTicket is basically your customs declaration. If you have a spouse or kid travelling with you, you can submit it altogether.
The form can be found on the website of the Dirección General de Migración (General Directorate of Migration) of the Dominican Republic. The form requires travelers to provide their name, date of birth, passport number, flight information, and intended dates of travel.
Passports and visa: U.S. citizens must have a valid U.S. passport to enter Dominica. No visa is required for stays less than 6 months if you have an onward or return ticket, confirmation of accommodation, and can produce evidence of your ability to maintain yourself.
