Your server mails the papers to your spouse or partner using regular first class mail. Fill out Proof of Service form. You can use Proof of Service by Mail (form FL-335). Keep the Proof of Service.

417.10. Proof that a summons was served on a person within this state shall be made: (a) If served under Section 415.10, 415.20, or 415.30, by the affidavit of the person making the service showing the time, place, and manner of service and facts showing that the service was made in ance with this chapter.

An example of a Proof of Service is a signed declaration from the person who served the documents, stating the name of the person who was served and the date, time, and manner of service. A Proof of Service may also include an affidavit, which is a sworn statement that the documents were properly served.

A summons may be served by personal delivery of a copy of the summons and of the complaint to the person to be served. Service of a summons in this manner is deemed complete at the time of such delivery.

The process server must fill out and sign the proof of service form, which shows how, when, and where the papers were served, and who received them. The server then gives you the proof of service. Make a copy of the proof of service. Take both the original and the copy to the court clerk to file.

Section 1008 - Service; Proof of Service (a) Proof of Service of papers shall be a Declaration stating the title of the paper Served or filed, the name and address of the person making the Service, and that he or she is over the age of 18 years and not a party to the matter.
