417.10. The process server must fill out and sign the proof of service form, which shows how, when, and where the papers were served, and who received them. The server then gives you the proof of service. Make a copy of the proof of service. Take both the original and the copy to the court clerk to file.
Section 1008 - Service; Proof of Service (a) Proof of Service of papers shall be a Declaration stating the title of the paper Served or filed, the name and address of the person making the Service, and that he or she is over the age of 18 years and not a party to the matter.
