Form 1099-R is used to report the distribution of retirement benefits such as pensions, annuities or other retirement plans.

Reportable disability payments made from a retirement plan must be reported on Form 1099-R. Generally, do not report payments subject to withholding of social security and Medicare taxes on this form. Report such payments on Form W-2, Wage and Tax Statement.

Members with at least twenty years of creditable service are eligible to retire at any age. A member's monthly retirement allowance is calculated using the member's age at retirement, years of creditable service, and amount of the member's high, three-year average salary.
