The City of Phoenix does not have or issue a general business license. Only certain activities are regulated and require a license or require a recommendation/approval from the City.
Proof of Service in Arizona The affidavit must state where, when, and how service was attempted, what efforts were made, and that the server is a registered process server for the state of Arizona. If service was completed, the document must be signed under penalty of perjury and returned to the plaintiff.
To begin the registration process, visit and click the tab titled Want to do business with the State? You will need your entity's exact legal name as registered with the federal government and your federal tax identification number to complete the process. Registration takes about ten minutes.

General The City of Phoenix requires a license from all businesses that have a privilege (sales) or use tax liability. We do not require a license for businesses without a tax liability, except for business activity subject to a Business / Regulatory license.

Contact the Clerk of the Superior Court for the county in which the divorce was filed in. Divorce decrees filed before 1950 may be held by the Arizona State Archives, depending on the county. Contact the Clerk of the Superior Court for the county they were filed in.
