The NYCERS Form 361, otherwise known as the Preliminary Retirement Estimate Application, is a form that is used by members of the New York City Employees' Retirement System (NYCERS) to request a preliminary estimate of their retirement benefits.

Withdrawal. If you leave public employment with less than ten years of service credit, you may end your NYSLRS membership and request a refund of your contributions. Withdrawing your contributions terminates your NYSLRS membership and you would become ineligible for any Retirement System benefits.

To withdraw your membership, sign in to your Retirement Online account, go to the 'My Account Summary' area of your Account Homepage and click “Withdraw My Membership.” You can also apply by mail by submitting the Withdrawal Application (RS5014).

Form #542. You may withdraw your Service Retirement Application by filing this form with NYCERS up to the day before your effective date of retirement. If your request is received on or after your retirement date, it is denied.

To transfer a membership to NYSLRS, you first must submit a transfer request to your other retirement system. When we receive your membership information from the other retirement system, we will compare your date of membership in NYSLRS with your date of membership in the other system.
