Submit a copy of a current state or federally issued photo ID with whatever documentation is changing the name (marriage certificate, legal name change from the court) to licensing@schools.utah. Please include your CACTUS number and any preference on how the name should appear on your license.
To create your USIMS account, log into and click the “Sign Up” button. Information entered in this system is confidential. For first-time users, you will be assigned a CACTUS ID (license) number at the completion of your information entry.
You will need to apply for a teaching license from your new state and have your credentials evaluated. If your credentials are found to be equivalent to the new state's typical requirements, you will be issued a new license. If they are not, you will be informed of any additional requirements.
