Possible Complications Some people may have a condition shortness of breath and low arterial blood oxygen levels when sitting or standing. This is called platypnea-orthodeoxia. This is rare. Rarely, people with PFOs may have a higher rate of a certain type of stroke (called paradoxical thromboembolic stroke).
What are the symptoms of patent foramen ovale? Most children have no symptoms from a PFO unless they also have other heart defects. In rare cases, a baby with a PFO will develop a bluish skin tone (cyanosis) when straining or crying.

An echocardiogram can be done to diagnose a PFO. If the PFO is not easily seen, a cardiologist can perform a "bubble test." Saline solution (salt water) is injected into the body as the cardiologist watches the heart on an ultrasound (echocardiogram) monitor.
A patent foramen ovale may be difficult to confirm on a standard echocardiogram. Your provider may recommend this test to get a closer look at the heart. A transesophageal echocardiogram takes pictures of the heart from inside the body. It's considered the most accurate way to diagnose a patent foramen ovale.
The gold standard for diagnosing PFO is contrast enhanced transoesophageal echocardiography (TOE) (fig 1).
