CHANGES TO CAUSE OF DEATH If additional medical information or autopsy findings become available that would change the cause of death originally reported, the original death certificate should be amended by the certifying physician by immediately reporting the revised cause of death to the State Vital Records Office.
O A completed VS24A (Physician/Coroner's Amendment) form. Amendment forms may be used to correct errors and/or add information that was not known at the time the vital record is registered. The amendment form is filed with and becomes a part of the record to which it pertains.
We usually order amended copies from the county health department. We'll have to wait for the state to mail the registered paper copy to the county before they can provide us with the new, amended, death certificate. We tell our families to expect to wait 6-8 weeks for the whole process.
How do I fill this out? Collect accurate vehicle information and documents. Identify errors in the current affidavit that need correction. Fill in the correct details in the specified fields. Review the affidavit for completeness and accuracy. Sign and date the affidavit to validate the corrections.
You need a court order if: There are questions about who is a parent of the child. The parties do not agree on who is a parent of the child. The parties want to add or remove parent(s) on the child's birth certificate.
Although states can offer their own variations, most comply with the U.S. Standard Death Certificate issued by the CDC's National Center for Health Statistics (NCHS).
