A Certificate of Service is a document confirming that an individual or company was served legal documents. It is an important document because it needs to be filed with the court in order to prove that the person or company was served and was given the opportunity to respond.

Use this form to certify to the court that documents were mailed or hand delivered to a party in a case.

The certificate of service must state that you served every other party, how you served them (generally, by mail or by the court's electronic filing system), and on what date you served them. If you fail to do this, your filing may be rejected and your case may be dismissed.

The clerk shall not accept for filing any pleading or other paper requiring service, other than an original pleading, unless it is accompanied by an admission or waiver of service or a signed certificate showing the date and manner of making service. A certificate of service is prima facie proof of service.

A certificate of service is a signed certification that, at the time of filing, the filing party served a copy of the document on all parties to the appeal. A certificate of service in an appeal requires more information than is required of a certificate of service in a trial court filing.

A Certificate of Service is used to prove to the Court that copies of pleadings such as motions, and discovery requests, have been mailed or hand delivered to the other parties in the lawsuit.

A sample certificate of service that counsel of record may use to prove service of interlocutory documents in civil actions in the Pennsylvania courts of common pleas, including service by hand delivery, leaving the documents at an authorized location, mail, fax, and electronic delivery.
