CJA Form 30 Death Penalty Proceedings: Appointment of and Authority to Pay Court-Appointed Counsel is a form used in federal death penalty cases. The form is used to appoint counsel for a defendant in death penalty proceedings and to authorize the payment of fees and expenses to the counsel. There are two types of CJA Form 30 Death Penalty proceedings: Appointment of and Authority to Pay Court-Appointed Counsel: the initial appointment and the reappointment. The initial appointment is used to appoint counsel to a defendant for the first time. The reappointment is used to reappoint counsel to a defendant for subsequent proceedings. The form includes information such as the name of the defendant, the name of the court, the name of the counsel, the amount of fees and expenses to be paid to the counsel, and the period of time for which the counsel is appointed.
