The California Civil Case Cover Sheet Addendum is a form used by the court to provide additional information about a case. It is generally filed with the Civil Case Cover Sheet (form CM-010) and must be filed in all civil cases except small claims cases. This addendum is used to provide such information as the estimated trial time and the names of any additional parties to the action. The California Civil Case Cover Sheet Addendum also requires the parties to indicate whether a jury trial is being requested and the amount of damages being sought. There are three types of California Civil Case Cover Sheet Addendum: Form CM-010A for cases with jury trials, Form CM-010B for cases without jury trials, and Form CM-010C for cases with multiple defendants.
