This Revocation of Statutory Power of Attorney for Health Care form is a revocation of the authority and power granted in Form NC-P014, which gives the person you designate as your agent/attorney in fact the power to make health care decisions for you. You may revoke a health care power of attorney at any time, so long as you are capable of making and communicating health care decisions. You may exercise this right of revocation by executing and acknowledging an instrument of revocation, by executing and acknowledging a subsequent health care power of attorney, or in any other manner by which you communicate an intent to revoke.
