A Confidentility and Security Statement and Confidentiality Statement of Understanding to be used by organization/company whose employees will be collecting and/or using data as part of a specific study that is funded by the Centers for Disease Control and Prevention (CDC)/National Center on Birth Defects and Developmental Disabilities (NCBDDD). The Confidentiality and Security Statement is intended to be the minimul standards for any and all sites participating in the study; and if so desired, additional policies as long as they are in compliance with the home institution's policies and guidelines.
