This form is used by an individual that sells or furnishes material or labor for the original construction of a single of a single or double family dwelling for the intended occupancy of the owner of the property where construction is taking place. It serves to put the owner on notice that a lein could result from the furnishing of labor and materials. The party issuing the notice is required to supply the property owner with a copy of this notice and file the notice in the recorder's office within sixty (60) days from the date of the first delivery or labor performed.
