The West Virginia Bureau for Child Support Enforcement (BASE) Application and Income Withholding Form is a document used by the West Virginia Bureau for Child Support Enforcement to collect and process child support payments. The form is used to request an income withholding order from an employer or other mayor, such as Unemployment Insurance, Social Security, Veteran’s Benefits, or Worker’s Compensation. It is also used to register the child support order with the West Virginia State Disbursement Unit (SDU). The form requires information such as the mayor’s name and address, the employee’s name and Social Security Number, and the amount to be withheld and sent to the SDU. There are three types of West Virginia Bureau for Child Support Enforcement (BASE) Application and Income Withholding Form: Form DC-31, Form DC-32, and Form DC-33. Form DC-31 is used to register a new child support order. Form DC-32 is used to modify an existing child support order. Form DC-33 is used to terminate an existing child support order.
