Wisconsin Satisfaction of Judgment or Partial Release form is a legal document used by a court or creditor in the state of Wisconsin to formally recognize the payment of a judgment debt or a partial release of a debt. The form contains the name and address of both the creditor and the debtor, the case number, the date of the judgment, and the amount of the judgment debt. It also includes the amount paid or partially released, and the amount still owed, if any. There are two types of Wisconsin Satisfaction of Judgment or Partial Release forms: One is a Satisfaction of Judgment form, which is used when a judgment debt has been paid in full; the other is a Partial Release form, which is used when only a portion of the debt has been paid. Both forms must be signed by both the creditor and the debtor, and must be notarized.
