Wisconsin JC-1641, Order for Hearing and Investigation/Screening, is a form used by Wisconsin courts to order a hearing or investigation/screening for a juvenile who is alleged to have committed a delinquent or unruly act. The order can be issued upon application of the district attorney or other prosecuting attorney, or upon request of a parent, guardian, or other person having legal custody of the juvenile. There are two types of Wisconsin JC-1641, Order for Hearing and Investigation/Screening: (1) Juvenile Delinquency Hearing, and (2) Juvenile Unruly Hearing. The Juvenile Delinquency Hearing is used to decide whether a juvenile has committed a delinquent act, and the Juvenile Unruly Hearing is used to determine whether a juvenile has committed an unruly act. The order requires the court to set a time and place for the hearing and to provide notice of the hearing to the juvenile and their parent or guardian. The order may also authorize the court to investigate the juvenile’s home environment and to require the juvenile to submit to a mental health screening.
