The Wisconsin Application for Child Support Services is a form used by the Wisconsin Department of Children and Families (DCF) to help establish, modify, or enforce a child support order. This application is available online, and can be used to apply for services in the following areas: establishing paternity (mother and/or father); establishing, modifying, or enforcing a child support order; establishing, modifying, or enforcing a health insurance obligation; and collecting and distributing child support payments. The application is typically used by custodial parents, non-custodial parents, and guardians of a minor child. It is also used to provide information to the Wisconsin Office of Child Support so that they can make an informed decision about a child support case. The application must be completed and submitted to the Wisconsin Office of Child Support for processing.
