This process is used when the contact information, or the individual assigned as the POC, needs to be changed for any reason. There are two types of Washington Change Assignment of Primary Point of Contact: Contract Changes and Contract Assignments. Contract Changes involve changing the POC information associated with a contract, such as the name, contact information, or other details associated with the POC. Contract Assignments involve assigning a new individual as the POC for a specific contract. In either case, the Washington Change Assignment of Primary Point of Contact form must be completed and submitted to the Washington State Office of Procurement for approval.
