Washington Notice of Occupational Disease or Infection is a form issued by the Washington State Department of Labor & Industries that employers must provide to their employees when the employee has been exposed to a hazardous or infectious agent or substance at their workplace. This form must be completed and sent to the Department of Labor & Industries within 15 days of the employee's first report of illness or injury. There are two types of Washington Notice of Occupational Disease or Infection: 1. Notice of Occupational Disease or Infection — This form is used to inform the employee of the potential risks of occupationally-acquired disease or infection, and provides information about the employee's legal rights and remedies. 2. Notice of Occupational Injury or Illness — This form is used to inform the employee of the potential risks of occupationally-acquired injury or illness, and provides information about the employee's legal rights and remedies.
