Washington Form 9. Petition for Review is a form used to appeal a decision from a state administrative agency in Washington. It is used to request a review of an agency's decision by the Washington Court of Appeals. There are two types of Washington Form 9. Petition for Review: a Petition for Discretionary Review and a Petition for Direct Review. The Petition for Discretionary Review is used to appeal an agency decision where the agency has discretion to decide the case. The Petition for Direct Review is used to appeal an agency decision where the agency does not have discretion to decide the case. The Petition for Review must be filed within 30 days of the agency's decision and must include information such as the name of the agency, the date of the decision and the specific grounds for the appeal.
