The Vermont Wage Statement (For Injuries On Or After 07-01-2008) is a document issued by the Vermont Department of Labor to employees who have sustained a work-related injury or illness on or after July 1, 2008. It outlines the employer’s responsibility for paying wages to an injured worker for the time missed from work, as well as any other benefits the worker may be entitled to. The statement also includes information about any deductions from wages. There are three types of Vermont Wage Statement: (1) Form C-1, which is used for injuries that occurred before July 1, 2008; (2) Form C-2, which is used for injuries that occurred on or after July 1, 2008; and (3) Form C-3, which is used for injuries that occurred on or after July 1, 2018.
