Tennessee Apostle / Authentication Request Form is a document issued by the Tennessee Secretary of State which attests to the authenticity of a document or document copy. It is a form of authentication that confirms the document is genuine and legally binding. The Tennessee Apostle / Authentication Request Form is used as evidence of the document’s validity when it is presented to foreign governments or international organizations. There are two types of Tennessee Apostle / Authentication Request Forms: Apostle Request Form and Authentication Request Form. An Apostle Request Form is used for documents that will be sent to a foreign country that is a member of The Hague Convention. The Authentication Request Form is used for documents that will be sent to a foreign country that is not a member of The Hague Convention. The Tennessee Apostle / Authentication Request Form must include the following information: the full name of the document recipient, the address of the document recipient, the type of document, a detailed description of the document, the name of the agency issuing the document, the date of issuance, the signature and title of the issuing official, the number of documents being authenticated, the fee for authentication services, and the document’s return address.
