A Pennsylvania Notice of Appeal is a document used in the state of Pennsylvania to initiate an appeal of a court decision. It is filed with the court to request a review of a previous ruling. There are three types of Pennsylvania Notice of Appeal: Civil Notice of Appeal, Criminal Notice of Appeal, and Appellate Court Notice of Appeal. The Civil Notice of Appeal is used to appeal a decision from a civil court, such as a small claims, family, or other civil court. The Criminal Notice of Appeal is used to appeal a criminal court ruling, such as a conviction or sentence. The Appellate Court Notice of Appeal is used to appeal a decision from the Pennsylvania Superior Court or the Pennsylvania Supreme Court. Each type of Notice of Appeal must include the name and address of the appellant, the name of the court from which the appeal is being taken, the court’s docket number, a designation of the type of appeal being taken, a statement of the relief requested, and the signature of the appellant or his/her attorney.
