Oregon Prosecuting Attorneys Notification of Compliance — Adult is a form that must be completed by individuals convicted of a crime in Oregon, and who are now under the jurisdiction of the Oregon Department of Corrections (DOC). This form is intended to ensure that prosecuting attorneys across the state are informed of the status of a convicted individual’s sentence. There are two types of Oregon Prosecuting Attorneys Notification of Compliance — Adult: one for individuals who are being supervised by the DOC, and one for those who have completed their sentence. Both forms require the individual to provide information such as name, date of conviction, and the DOC's supervision status. The forms must be signed and returned to the prosecuting attorney in the county where the individual was convicted. Once completed, the form is used to inform the prosecuting attorney of any changes to the individual's sentence or release status.
