The Oregon Certificate of Service is a document issued by the Oregon Judicial Department that attests to the legal service of process in a civil case. There are two forms of Oregon Certificate of Service: the Summons and Complaint and the Original Notice. The Summons and Complaint form is issued when a Summons and Complaint has been served, while the Original Notice form is issued when an Original Notice has been served. Both forms must be signed by the process server, the person who served the documents, and the recipient, or the person being served. The Oregon Certificate of Service must then be submitted to the court as evidence of service.
