Oregon Declaration Non-Military, Minor or Incapacitated Person is a legal document used in the state of Oregon by individuals who are unable to make decisions for themselves due to mental or physical incapacity. The document allows the individual to appoint a representative to make decisions concerning their finances, health care, and other important matters.
