Ohio Self-Insured Employers Certification of Assignment After Initial Allowance is a document used by Ohio-based self-insured employers to certify that an injured worker has been assigned to a specific provider for medical care after an initial allowance of benefits has been processed. The document is used to confirm that the employer has taken all necessary steps to ensure that the injured worker is receiving proper care and that all costs associated with the necessary medical care are being covered.
