A New York Notice of Missing or Mutilated Vehicle Identification Number (VIN) is a document that is used to inform the New York State Department of Motor Vehicles (DMV) that a vehicle's VIN is either missing or has been mutilated. This document is filed by the owner of the vehicle or their legal representative. There are two types of New York Notice of Missing or Mutilated Vehicle Identification Number: Form MV-41A and Form MV-41B. Form MV-41A is used when the vehicle's VIN has been mutilated and is unreadable. The owner of the vehicle must provide an explanation for how the VIN was mutilated. The DMV will then issue a new VIN to the vehicle. Form MV-41B is used when the vehicle's VIN is missing. The owner of the vehicle must explain how the VIN is missing, as well as provide proof of ownership. The DMV will then issue a new VIN to the vehicle.
