There are two types of New York Claimants Authorization To Disclose Workers Compensation Records: the Authorization to Disclose Information, and the Authorization to Disclose Medical Information. The Authorization to Disclose Information allows the NSW to release information such as the claimant's name, date of birth and Social Security number, contact information, and claim status. The Authorization to Disclose Medical Information allows the NSW to release medical information related to the claimant's injury, such as diagnoses, treatment history, and medical bills.
