The New York Mediation Initiation Form is a document used to initiate the mediation process in New York State. This form is used in civil litigation cases, and in family and divorce proceedings. It contains information about the parties involved, such as contact information, address and phone numbers, as well as a brief description of the dispute. It also includes a declaration of the parties' agreement to participate in the mediation process. There are three types of New York Mediation Initiation Form: 1) the Joint Petition for Mediation; 2) the Individual Petition for Mediation; and 3) the Notice of Mediation. The Joint Petition is used when both parties agree to mediate the dispute. The Individual Petition is used when one party is requesting mediation, but the other is not. The Notice of Mediation is sent to the parties when the court has ordered mediation.
