A New York Miscellaneous Case Information Sheet (MCI) is a form used to provide information about a particular case to the court. MCI forms are used to track all activities related to a court case, including filing of documents, scheduling of hearings, and other case-related activities. The forms are available for download from the court's website and must be completed and submitted for every case. There are two types of New York Miscellaneous Case Information Sheets: the General Form and the Summary Form. The General Form contains detailed information such as the case number, case title, parties involved, court location, and case status. The Summary Form provides a summary of the case without the detailed information. Both forms must be submitted to the court for each case.
