Commercial Motor Vehicle Tax (CMVT) Exemption Affidavit The New York Commercial Motor Vehicle Tax (CVT) Exemption Affidavit is a form required by the New York State Department of Taxation and Finance to be completed by commercial motor vehicle operators in order to receive a tax exemption from the CVT. The affidavit must be completed and signed by the owner of the vehicle, or an authorized representative, and must include details of the vehicle's use and purpose for the exemption. There are three types of New York Commercial Motor Vehicle Tax (CVT) Exemption Affidavits: 1) for interstate commerce; 2) for intrastate commerce; and 3) for rental vehicles. The affidavit must be completed accurately and in full in order for the operator to receive the exemption.
