The Nevada ADOPTED Application (Electronic) is a secure online form used to request a copy of an adult adopted's original birth certificate. It is an easy way for an adopted to obtain a copy of their original birth certificate without having to go through the more lengthy process of obtaining it from the court or through the state of Nevada Department of Health and Human Services. The application is simple: adopters must provide their name, date of birth, and contact information. They can also include any additional information they wish to provide, such as a current address, parents’ names, and any other identifying information. The Nevada ADOPTED Application (Electronic) has two different forms: one for those born in Nevada and one for those born outside of Nevada. The form for those born in Nevada is designed to provide birth certificate information from the state of Nevada. The form for those born outside of Nevada is designed to provide birth certificate information from the state where the adopted was born. Both forms require the adopted to provide a valid government-issued photo ID. Once the application has been completed and submitted, the adopted will receive an email notification that their application has been received and is being processed. The adopted will then receive their original birth certificate within six to eight weeks.
