The form includes the names of the debtor and the creditor, the debtor’s bankruptcy case number, and the bankruptcy court where the case was filed. There are two types of Nevada Notice of Bankruptcy Nonrefillablele Form: Chapter 7 and Chapter 13. The Chapter 7 form is used when the debtor is filing for liquidation, while the Chapter 13 form is used when the debtor is filing for reorganization.
