The New Jersey Appellate Civil Case Information Statement (ACID) is a form used to provide the New Jersey Appellate Division with initial information about a civil case. This statement is required to be filed when a civil case is appealed to the Appellate Division from a lower court. The information requested on the form includes the names of the parties, the nature of the case, the lower court’s decision, and the reasons for the appeal. There are two types of ACID: the short form and the long form. The short form is used when the appeal involves a single issue and fewer than five parties. The long form is used when the appeal involves multiple issues or more than five parties. Both forms must be completed and submitted to the Appellate Division prior to the filing of the appeal.
