The form must be signed and sworn to by the attorney making the request. There are two types of New Jersey CJA Form 26: Supp. Information Statements for Compensation Claims in Excess of the Stat. Case Compensation Maximums. The first is used for individual cases and the second is for multiple cases. For individual cases, the form requires the attorney to provide the court with information such as the nature of the case, the number of hours spent on the case, the complexity of the case, the results obtained, and the amount of compensation requested.
