The patient/client specifically states if they will accept visitors within the normal visiting times and/or arrangements or the patient/client can determine to limit visitors. Patient/Client will also specify telephone use, medical records disclosure and the patient/client will also confirm understaning and acknowledgement of compliance with state and federal laws and agrees that they reserve the right to change the choices at anytime during treatment.
