The Missouri Quarterly Contribution and Wage Report is a document that employers in the state of Missouri are required to file with the Missouri Department of Labor and Industrial Relations in order to accurately report wages and contributions to the unemployment insurance program. This report is due within 20 days after the end of each quarter, and is used to calculate employers' contributions to the unemployment insurance program. There are two types of reports: the Quarterly Contribution and Wage Report (Form ES-2), used to report wages and contributions for all employees, and the Quarterly Contribution and Wage Report for Household Employees (Form ES-3), used to report wages and contributions for household employees. The report must include employee wages, hours worked, taxes withheld, and any contributions made to the unemployment insurance program. Employers must submit the report to the state agency using the approved forms and must maintain a copy of the report in their records.
