The Missouri Statement of Change of Registered Agent and/or Registered Office by a Cooperative Association is a form required by the Secretary of State's office to be filed by a cooperative association in order to change its registered agent and/or registered office address. The form must be signed and notarized by an officer of the association. There are two types of Missouri Statement of Change of Registered Agent and/or Registered Office by a Cooperative Association: one for changing the registered agent and another for changing the registered office address. Both forms require the name of the cooperative association, its current registered agent and office address, the name and address of the new registered agent and office address, and the name of an officer of the association who is authorized to sign the document. The form must be accompanied by a $10 filing fee.
