The Minnesota Confidential Document Cover Sheet — CHIPS (Form 11.3) is a form used to protect confidential information within the state of Minnesota. It is used to protect the identity and personal information of individuals involved in Child and Health Insurance Programs (Chips) and Medicaid. This form can be used to secure information for people who are applying for or receiving these benefits, and to protect the identity and personal information of those who may be affected by the application or program. It also serves to protect the confidentiality of all documents associated with Chips and Medicaid. There are two types of Minnesota Confidential Document Cover Sheet — CHIPS (ForChippersPs Privacy Form (Form 11.3a) and Medicaid Privacy Form (Form 11.3b).
