Minnesota Release and Authorization is a legal document that is used to provide authorization for the release of a person's private and/or confidential information in the state of Minnesota. There are two main types of Minnesota Release and Authorization: the Minnesota Release and Authorization for the Use of Personal Information and the Minnesota Release and Authorization for the Use of Credit History. The Minnesota Release and Authorization for the Use of Personal Information is a document in which an individual grants permission to a third party to obtain their personal information, such as name, birthdate, address, and Social Security Number. The Minnesota Release and Authorization for the Use of Credit History is a document in which an individual grants permission to a third party to obtain their credit report. Both types of Minnesota Release and Authorization must be signed by the individual and notarized in order to be valid.
