Maryland Petition for Appointment of Health Care Professionals is a legal document that must be filed in order to appoint a health care professional to provide medical care for an individual in the state of Maryland. It is typically used when an individual is unable to make decisions regarding their medical care on their own, or when they are unable to find a health care professional to provide care. The petition must be signed by the individual who is requesting the appointment, or their legal guardian. It must also be signed by two witnesses. There are three types of Maryland Petition for Appointment of Health Care Professionals: Appointment of a Health Care Agent, Appointment of a Guardian of the Person, and Appointment of a Guardian of the Estate. Appointment of a Health Care Agent allows an individual to make decisions regarding their medical care in the event that they are unable to do so themselves. Appointment of a Guardian of the Person allows a third party to make decisions regarding an individual’s health care, lifestyle, and well-being in the event that the individual is unable to do so themselves. Appointment of a Guardian of the Estate allows a third party to manage an individual’s financial affairs in the event that the individual is unable to do so themselves.
