Louisiana Disclosure of Ownership (Corp) is a legal document that must be filed with the Louisiana Secretary of State in order to incorporate a business. It is required by the state of Louisiana to ensure that the business owners are publically known and that the public can access the information. The document must include the name of the corporation, its registered office and agent, the names of the officers, directors, and shareholders, and the total number of outstanding shares of the corporation. There are two types of Louisiana Disclosure of Ownership (Corp): Initial Disclosure of Ownership and Annual Disclosure of Ownership. The Initial Disclosure of Ownership must be filed when the corporation is initially formed, and the Annual Disclosure of Ownership must be filed each year thereafter.
